245D service logs, written at the visit
Homemaking, companion and in-home respite logs are captured on the phone when the visit ends, signed on the spot, and billed from the same record.
What a 245D log holds
A service log for 245D Basic Support Services, from the field to the service line
Captured when the visit stops
Stopping a 245D visit on the mobile app opens the service log right away, while the details are still fresh.
Tasks, narrative and incidents
A task checklist for each service, a narrative, the goals worked on, and an incident section that asks for real detail when used.
Two signatures
The client and the staff member sign on the device before the log is submitted, with the name of whoever signed for the client.
Late entries need a reason
A log for a past date cannot be saved without a written reason, and those logs are counted separately on the dashboard.
The log is written where the work happens
Direct support staff should not be writing up a homemaking visit hours later from memory. When a worker stops a 245D visit in the AraSync mobile app, the app recognizes the service code and opens the matching service log before the visit is checked out.
- ✓The right log opens. The app matches the visit's service code and opens the homemaking, companion or respite log.
- ✓A task list per service. Each of the three services has its own checklist.
- ✓Drafts save as you go. Work in progress is saved every 30 seconds, so an interruption does not lose the write-up.
- ✓Times from the visit. The start time comes from the check-in and the end time from when the log is written. Both can be edited.
- ✓Checks before submit. At least one task, both signatures, and a real description when an incident is marked.
- ✓Log, then check out. The visit is checked out after the log is captured, so it does not close without documentation.

Basements and rural roads are part of the job
If the phone has no connection, the draft and the finished log are stored encrypted on the device. The submission and the checkout are queued and sent when the connection returns.

Three basic support services
Each billed in 15-minute units from the log itself
- ✓Homemaking. S5130 Homemaker services.
- ✓Companion. S5135 Adult companion care.
- ✓In-home respite. S5150 Respite care in the home.
Turning on the 245D program adds the three service codes and their permissions for your agency. Your team sets the payer rates.

Oversight from the office
Coordinators get a 245D program area with a dashboard, a client list, a tab for each service, documents, staff and reports. Supervisors see the same logs the workers filed.
- ✓Dashboard. Clients, logs filed this week, logs not yet billed, and late entries in the last 30 days.
- ✓A tab per service. Homemaking, companion and respite logs, with date and status filters.
- ✓Tied to a real visit. Picking a client and date lists that client's visits, so each log documents an actual visit.
- ✓Full log view. Tasks, narrative, incidents, both signatures and the linked service line in one place.
- ✓One log per visit. The same visit cannot be documented twice.
Fix a log without losing track of the claim
Submitting a log creates its service line in the same step. Corrections follow what has already been billed, and AraSync enforces that in the software.
- ✓A service line on submit. Units, rate and charge come from the log and link back to it.
- ✓The payer on the service date. The line uses the payer that was active on the date of service, not today's.
- ✓Editable until billed. Narrative, goals, incidents, tasks and times can be corrected. Changing the times recalculates the duration.
- ✓Locked once billed. A billed log cannot be edited or voided. Changes go through the billing adjustment process.
- ✓Void with a reason. Voiding records who, when and why, and voids the pending service line with it.
- ✓Times that make sense. The end must follow the start, durations over 24 hours are rejected, and log dates are stored so they do not shift a day across time zones.
